2018-19: our prescription benefits are managed by CVS/Caremark but you can use any pharmacy which is in network such as Stop and Shop or Walgreens.
Make sure you set up an online account with Caremark.
Call 1-800-318-2572 to request duplicate CVS/Caremark identification cards. After the employee places this call for duplicate prescription cards, it will take 8 to 11 business days before it will arrive in generic white envelope to employee’s home address on file.
please note: The membership number on the employee’s medical id card and employee’s prescription id card are the same membership number.
